GP Chronic Condition Management Plan (GPCCMP)
A GP Chronic Care Management Plan (GPCCMP) is a comprehensive document that details a patient's chronic conditions, care goals, treatment actions, necessary referrals, and a review schedule. It is designed to ensure coordinated, ongoing care for patients with long-term health conditions. Eligibility includes those with chronic conditions expected to last six months or more, or individuals with a terminal illness. By outlining a clear path for treatment and follow-up, the GPCCMP supports both patients and healthcare providers in managing complex health needs, improving patient outcomes, and fostering proactive, patient-centred care.
Men's Health
Men’s health is an essential focus of Amara Medical Geelong, encompassing proactive care for various conditions. These include regular prostate checks for early detection of prostate issues, including cancer, mental health support, sexual health concerns such as erectile dysfunction and sexually transmitted infections (STIs). Heart disease management is also critical, as men are at higher risk for cardiovascular issues. Women's Health

Family Health

Skin Cancer Checks
Regular skin checks and skin surviellance is important when we live in Victoria. With further training in skin cancer screening and surgeries, some of our Doctors are able to identify and treat skin cancers at Amara Medical and have relationships with local specialists to refer to if it requires intervention. We encourage all patients as part of their general health screening to have regular skin checks.
Mental Health
All our Doctors will treat mental health conditions. This could include referral to external providers and/or medication. Mental health care plans can be obtained from your GP.
Please note: We do not provide ADHD assessments and diagnosis.

Do I need a referral to see a specialist?
For most specialist appointments in Australia, a GP referral is required to access Medicare benefits. Our doctors can assess your needs during your consultation and provide a referral to the appropriate specialist if required. Referrals are typically valid for 12 months (or 3 months for a specific condition). Please book a standard consultation to discuss a referral — we cannot issue referrals via phone or online requests.How do I get a repeat prescription?
Repeat prescriptions require a consultation with your GP to ensure your medication remains appropriate and safe for your current health status. Our doctors are unable to issue prescriptions without a consultation. To request a repeat prescription, please book a short appointment — many of these can be conducted as a telehealth consultation if clinically appropriate.Is telehealth available at Amara Medical Geelong?
Yes, we offer telehealth consultations for selected appointment types, including repeat prescription reviews, mental health follow-ups, and certain chronic disease management reviews. Telehealth is conducted by video or phone call. Please note that some conditions and services require an in-person visit. When booking, indicate that you would like a telehealth appointment and our team will confirm eligibility.What should I bring to my first appointment?
For your first visit, please bring your Medicare card, any current medications or a medication list, your Health Care Card or Pension Concession Card if applicable, any previous test results or specialist letters relevant to your visit, and your immunisation history if available. Arriving 5-10 minutes early to complete any paperwork helps ensure your consultation starts on time.
