GP Chronic Condition Management Plan (GPCCMP)
A GP Chronic Care Management Plan (GPCCMP) is a comprehensive document that details a patient's chronic conditions, care goals, treatment actions, necessary referrals, and a review schedule. It is designed to ensure coordinated, ongoing care for patients with long-term health conditions. Eligibility includes those with chronic conditions expected to last six months or more, or individuals with a terminal illness. By outlining a clear path for treatment and follow-up, the GPCCMP supports both patients and healthcare providers in managing complex health needs, improving patient outcomes, and fostering proactive, patient-centred care.
Men's Health
Men’s health is an essential focus of Amara Medical Geelong, encompassing proactive care for various conditions. These include regular prostate checks for early detection of prostate issues, including cancer, mental health support, sexual health concerns such as erectile dysfunction and sexually transmitted infections (STIs). Women's Health
Women's health is a strong focus at Amara Medical providing all types of care relating to women's health needs. Contraceptive choices and options, mind health, reproductive management and menopause management. Family Health
All family members are welcome at Amara Medical Geelong. We provide a calm and safe environment for all your family health needs. Immunisations and childhood health assesments are provided at Amara Medical Geelong.
Skin Cancer Checks
Regular skin checks and skin surviellance is important when we live in Victoria. With further training in skin cancer screening and surgeries, some of our Doctors are able to identify and treat skin cancers at Amara Medical and have relationships with local specialists to refer to if it requires intervention. We encourage all patients as part of their general health screening to have regular skin checks.
Mental Health
All our Doctors will treat mental health conditions. This could include referral to external providers and/or medication. Mental health care plans can be obtained from your GP.
Please note: We do not provide ADHD assessments and diagnosis.

What are the consultation fees?
Consultations will be privately billed and the fees are set by each Provider depending on the appointment type and consultation length. Full payment is required at the time of consultation. All procedures and specialist GP services will be quoted prior to your appointment.
What should I do if I need medical help after hours?
If you are experiencing any medical emergency outside of business hours, please dial 000 or present to your nearest emergency department. How can I get a medical certificate for work or school?
A consultation with one of our GPs is required to obtain a medical certificate. Consultations can either be face to face or over the phone if you are eligible for a telehealth consultation.
How do you share important updates?
For more personal or urgent matters, our staff may contact you directly by phone.
For non-urgent updates such as results recall, seasonal health advice, flu clinic announcements, or policy changes, we may send SMS reminders or emails if you’ve provided consent.
How can I make a complaint if I’m unhappy with my care?
